The TAP Admissions Team helps young adults who were in Child Intervention to transition into the Transition to Adulthood Program. If you already have a caseworker before you turn 18, you don't need to apply. The Admissions staff will have three meetings with you, your caseworker, and your support network when you are 17, 17.5, and 18 years old. During these meetings, they will talk to you about your future hopes and dreams, what you want to do, and the kind of help you need.
If you are eligible for TAP, closed your file before, and want to access TAP services, you can send an email to [email protected] with your full name, date of birth, and where you live.
